• To be eligible to begin servicing this new route, you must submit a Proposal and Contract for Regular Service to become an Independent Contractor (IC) of USMDG.

• If you submit a proposal to USMDG and that proposal is accepted, at that time you will execute an Independent Contractor Agreement with USMDG.

In the event you ultimately execute an Independent Contractor Agreement with USMDG you should be aware of the following:

• USMDG WILL NOT withhold any federal, state or local payroll or income taxes from the compensation given to you. It will be your obligation to pay all federal, state and local taxes payable as a result of the compensation that you receive as an independent contractor of USMDG. We encourage you to consult with an accountant to assist you in understanding the benefits and costs associated with becoming an Independent Contractor. Many individuals like you have found significant profit in being an Independent Contractor.

• You may want to consider forming an entity such as a corporation or limited liability company to further your efforts in being a USMDG contractor. Please be advised that corporations and limited liability companies are legal entities that may result in additional requirements and responsibilities for your business. We encourage you to consult with an attorney or accountant before making the decision to form one of these entities.

• USMDG will require that you obtain liability insurance for yourself and those whom you may use your relief back-up (Affiliated IC).

• You will be required to find a back-up to assist you in servicing your route. However, since the relief back-up is your staff and only an Affiliate of USMDG. You are responsible for setting compensation, hours of work, and other conditions and terms of employment.

• If for some reason your vehicle is not available to service your route, you are responsible for obtaining a suitable vehicle from any source you choose.

Please complete the forms contained in on this website, as they are necessary to properly evaluate your offer. USMDG reserves the right to award this solicited service with or without discussion(s), therefore the information you supply may be the sole basis on which your offer is judged. All offers must be in accordance with all current state and federal laws.

Once the contract has been awarded to you, adjustments to the bottom line can be made only in writing between you and USMDG. If you do not understand the adjustment process please inquire. All Offerors submitting proposals will receive a written notification of contract award. The notification will identify the successful Independent Contractor and awarded contract value.
